Ithaca Chill Challenge (January 1, 2017)

In volunteering to participate in the 2017 “Ithaca Chill Challenge,” I hereby agree that this activity is and shall be at my own risk against all casualties to myself or my property and that I take all risks of any kind no matter how caused and I hereby release and discharge Ithaca Community Recovery, Inc., Ithaca Yacht Club, all service providers, sponsors and volunteers (hereinafter “the organizers”), and indemnify them of and from all actions, claims, and demands of every nature and kind whatsoever which I, or my heirs, executors, administrators or assigns may now or can at anytime thereafter have against the said organizers of the “Ithaca Chill Challenge” for or on account of any loss, damage or injuries caused by negligence, default, or misconduct of the organizers themselves, servants, agents, members or otherwise howsoever.

Additionally, I hereby authorize Ithaca Community Recovery, Inc. to use my name, picture, video image, photograph or quotation of my remarks for public information, fundraising purposes and future publicity of "Ithaca Chill Challenge " including news media.

Furthermore, I hereby attest I am 18 years of age or older.
