Waiver for Ithaca Chill Challenge (January 1, 2022)
In volunteering to participate in the 2022 “Ithaca Chill Challenge,” I hereby agree that this activity is and shall be at my own risk against all casualties to myself or my property. I hereby assume all risks of any kind no matter how caused, including the risk of serious injury or death, and I hereby release, discharge, and hold harmless Ithaca Community Recovery, Inc., Ithaca Yacht Club, and all associated service providers, sponsors and volunteers (hereinafter “the organizers”), and indemnify them of and from all actions, claims, and demands of every nature and kind whatsoever which I, or my heirs, executors, administrators or assigns may now or can at any time thereafter have against the organizers of the “Ithaca Chill Challenge” for or on account of any loss, damage or injuries caused in any way, including those caused by negligence, default, or misconduct of the organizers themselves, their servants, agents, members or otherwise howsoever, to the maximum extent possible.
Additionally, I hereby authorize Ithaca Community Recovery, Inc. to use my name, picture, video image, photograph or quotation of my remarks for public information, fundraising purposes and future publicity of "Ithaca Chill Challenge " including news media.
Furthermore, I hereby attest I am 18 years of age or older.
This year, the Chill Challenge will take place during the global COVID-19 pandemic. I attest that I am not experiencing any of the symptoms associated with the novel coronavirus, that I have not been exposed to someone with a confirmed or suspected infection with the novel coronavirus in the past 14 days, and that I will comply with all the safety requirements of the Chill Challenge, including wearing a mask to the event, keeping a safe social distance from others at the event, washing and disinfecting my hands, and any other requirements necessary for public safety and comfort. If I will be participating in person at the Chill Challenge, I hereby assume the risk of infection with the novel coronavirus. I recognize that this virus is highly contagious and infection is a possibility just as with any social gathering.
* Symptoms of the novel coronavirus include but are not limited to: Fever, Shortness of Breath, Dry Cough, Runny Nose, Sore Throat, Loss of Taste or Smell.
